Description

A CAHA Forum addressing community collaboration to facilitate the integration of physical health, mental health, substance use and wellness and prevention services in the Capital Area region.

The Forum will feature presentations on integration models in other states, integration efforts underway in Michigan, and existing, viable models to integrate physical health and behavioral health services in the Capital Area. Two panels will explore the provider experience with integration models in the region and the ways in which health systems and organizations can work toward providing integrative care to individuals with complex needs.

The program is open to all members of the community and will be of interest to clinical and administrative professionals working for health clinics, health systems, long term care facilities, social services agencies, public health agencies, primary care and specialty medical groups, payers, employers and school based clinics.

Continuing Nursing Education (CNE) Contact Hours: 3.66 Contact hours
To receive CNE contact hours and a certificate, the criteria for successful completion includes the participant to be present at the entire event, verify attendance and complete an evaluation distributed electronically after the event.
Capital Area Health Alliance will distribute the post-event evaluation in Survey Monkey to the email address provided at registration. Michigan State University College of Nursing will distribute a CNE certificate separately when the evaluation is completed.
Michigan State University College of Nursing is approved as a provider of continuing nursing education by Ohio Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. (OBN-001-91).
